Your Role

As your patient progresses through the process, you will be asked to provide the following supporting information:

• Copies of your treatment notes.
• Referrals to specialists and diagnostic testing, when needed, to support your patient’s case.
• Completion of a disability questionnaire or other statement related to your opinion regarding your patient’s inability to work.

Attorney fees for Social Security disability cases are only paid if the claimant is awarded benefits. The amount of these fees is based on a percentage of the claimant’s retroactive benefits and is limited by the Social Security Administration.
